Fixed term contract/secondment opportunity for 12 months
The NNUH Cancer Services team are proud to be working in partnership with Macmillan and the Norfolk & Waveney STP to deliver a Cancer Care Navigator Service
As a Cancer Care Navigator you will be uniquely positioned to promote self-management and provide general information and support about cancer and cancer services, to enable people to navigate the health and social care system and make choices that are best for their cancer and their life. A key aspect of the role involves daily and direct (e.g. face to face and telephone) communication with patients, relatives, carers, and other health and social care professionals.
You will be undertaking Holistic Needs Assessments and care planning following a patient's cancer diagnosis. Following this you will be signposting to support and referring to appropriate services, as well as offering emotional support. The information and nature of communication is sensitive. Communication in this context requires a high degree of empathy, understanding, diplomacy, confidentially and integrity.

Main duties of the job
The role offers the opportunity to work in different locations and develop different styles of care. You will be expected to provide telephone support from the office at Rouen Road and deliver face to face support from the Information and Support Centres at the NNUH or Cromer Hospitals. You will also be required to support with the running of health and wellbeing events, internally and externally
You will provide support for the person living with cancer so that they access the right help at the right time, providing a seamless experience of care. Checking that the person can self- manage and provide a flexible and personalised response to any changes needed. You will help people living with cancer to access referrals and/or signposting to local supportive services that can meet an individual's range of personal needs.
You will have the opportunity to work within an exciting and dynamic team to help build the Patient Cancer Information and Support services within the NNUH trust to provide the right information, at the right time for each cancer patient to ensure individual needs are met.
Informal visits to the Cancer Care Navigator team or the Macmillan Information Pod or the North Norfolk Macmillan Information Centre (Cromer Hospital) are welcomed and encouraged prior to interview.

- Following cancer diagnosis, ensure every patient detail is added to the project database, to enable the support of all patients from the start of their cancer journey. Record all patient contacts with Somerset Cancer Registry
- Provide support both via the telephone and in the Cancer Information Services in Cromer and NNUH. Travel to these locations is required.
- Advise patients on individual self-care management principles and provide consistent planned aftercare to reinforce and further promote this information.
- To maintain confidentiality at all times and work within the Trusts policies and procedures.
- To be responsible for complying with Trust Infection Control policies and clinical guidelines.
- To prepare and actively participate in Trusts annual Performance Development Review process and progress reviews.
