The Virtual Ward is an exciting addition to the way we care for patients. Patients are cared for at home using wearable technology. Virtual ward nurses need to be dynamic, forward-thinking individuals with great problem solving and IT skills. The role requires flexibility of day and night shifts and the ability to occasionally work from home, so candidates will need a secure, confidential space to talk to patients.
Communication is key in virtual nursing, and we use a variety of communication methods and tools with both our and across the multidisciplinary team.
The service is continuously evolving so candidates will need to be adaptable to change and able to contribute to any new initiatives to improve patient care.

Main duties of the job
- Main duties include managing all aspects of care for a group of patients, from assessment and education prior to transfer to the Virtual Ward to their time on the ward and then discharge from the ward. Nurses are responsible for the assessment and monitoring of patients, using various methods from face to face assessment and video calling. Patients are presented on a virtual ward round daily and plans of care updated accordingly. The successful candidate will be responsible for checking the patients medications on EPMA and documenting that they are self- administered (unobserved) where appropriate and escalating any discrepancies.
- Work with all members of the Virtual Ward clinical team within the multi-disciplinary framework. Maintain a high standard of patient care under the direction of the Virtual Ward Sister/Charge Nurse. Presenting patients on the daily ward round and carrying out planned care.
- Manage a case load of patients who are at home, assessing patients, planning care and liaising with the multidisciplinary team to provide the best and most appropriate care for patients. Being fully responsible for all aspects of patient care and record keeping.
