Job summary
The Specialist Palliative Care team compromises of Clinical Nurse Specialists and Specialist Doctors. The specialist nurses work throughout the adult wards supporting patients with specialist palliative care needs (regardless of diagnosis) by addressing symptom control and offering psychological support to patients and their families. The team has an AP in post who supports fast track discharges who is working 3 days a week this post will be seeking to ensure we can offer five days a week support to fast track discharges
The team works 7 days a week and in the future the discharge AP may need to cover weekends. The service is covered 9 - 5 as a face to face service. This post is mainly to cover Thursday and Friday with flexibility throughout Monday - Friday as required. The working hours are 9 - 5.
Main duties of the job
Main responsibilities include:
- Support the discharges of patients who are being discharged rapidly for end of life care - ensuring all details of the discharge are and have been addressed to ensure seamless care as the patient transitions from one clinical area to home or another clinical area eg PBL/hospice in another part of the country/nursing home
- Act as a resource and support for the Specialist Discharge Nurses, Specialist Palliative Care Nurses & Doctors and Discharge Coordinators, working collaboratively with the voluntary agencies to support a safe and timely discharge.
- To act as the patient's advocate during palliative discharge planning
- Be able to support the finer details of the discharge and liaise with e family and patient and colleagues in the community/other hospitals as required to ensure a seamless transition of care.
- Support education as required.
- Complete data and spreadsheets as required.
- Communicate with all ward staff.
- Managing a caseload of patients who need specialist support for their discharge.
- Support patients with nursing care if required
