We have an exciting opportunity for a Clinical Nurse Specialist working as part of the dynamic new East Genomic Medicine Service Alliance (GMSA)set up at NNUH. East GMSA has been set up to ensure all eligible patients across the East Midlands and East of England can access and benefit from appropriate genomic tests when required, ensuring high-quality and personalised treatment. Covering the same catchment area as East Genomic Laboratory Hub, the East GMSA brings together the vital multi-disciplinary clinical leadership and other operational and digital functions that are necessary to embed genomic medicine into mainstream clinical care.

A key priority East GMSA is to support the NHS Long Term Plan ambition of increasing identification of Familial Hypercholesterolaemia (FH) whilst working with NHS England and NHS improvement and the other six GMS Alliances. This post is part of a national initiative, funded by NHS England and NHS Improvement through the East GMSA to implement FH pathways in primary care.

Main duties of the job

This is an interesting opportunity and a new post to support a 1 year pilot project to implement screening for FH in primary care. The screening programme will utilise innovative FH case finding informatics tools to screen primary care records for individuals at high risk of FH. You work will be vital in helping to support the aim of supporting primary care teams to increase the detection of FH in the community, in line with the NHS Long Term Plan commitment to identify 25% of the expected FH patients within 5 years.

This post will support the identification of high risk individuals in a small number of GP practices within NHS Norfolk and Waveney Clinical Commissioning Group (CCG). Following identification, patients at high risk will be invited to provide further medical details, a family history and be provided with information about the screening programme. They will be offered genetic testing for FH after providing consent. Those diagnosed with FH will be referred to local specialist lipid services. This gives you the chance to be involved in developing a new primary care pathway for FH.

The post holder will work with the project leads and local GPs to plan, implement and evaluate the new screening pathway. They will also be linked closely to Lipid Clinics and the genetic diagnostics laboratory commissioned to provide the testing service for FH.

  • The post will be based at Norfolk and Norwich University Hospitals NHS Foundation Trust (NNUH) and locally appointed GP surgeries. On occasions, the patient contact and consenting process may need to be undertaken virtually so a degree of flexibility would be advantageous.
  • The post holder will be expected to demonstrate professional initiative, independence and autonomy working as a nurse with the clear goal of initiating and delivering a primary care screening service to improve identification of patients at high risk of Familial Hypercholesterolaemia (FH)
  • Work closely with the locally based IT support services and the host organisations IT department to enable upload of patient information at point of direct patient contact.
  • Work with primary care staff, consultant physicians, consultant nurses, nurse specialists and other related professionals to develop high levels of care for patients with suspected and diagnosed FH
  • Implement a screening strategy to identify individuals at high risk of having FH, provide disease specific information, obtain consent for and arrange genetic testing.
  • The post holder will be able to learn and demonstrate knowledge of FH and its implications for the development of heart disease.
  • To be part of a team implementing a screening service for the identification of individuals at high risk of developing FH
  • The post holder will be expected to learn how to deliver potentially sensitive information to patients and their families in relation to FH and its implications
  • To undertake diagnostic testing for FH using genetic and biochemical tests. This will involve discussing information in order to inform and counsel individuals on the significance of such investigations both for the individual and their family members
