Job summary

We are looking for a confident and experienced individual to join within the Staffordshire Reconnect Service. The Reconnect service sits within our wider Health and Justice service, supporting people with needs and vulnerabilities in all aspects of the criminal justice system. Reconnect reaches into 5 of the prisons within Staffordshire, undertaking holistic needs assessments and engagement work in order to support them on release from prison to get their health and social care needs met within the community and reduce the risk of reoffending. Reconnect sits within a national network of reconnect services, and works both within and outside of the prison estate. IOH . Our Navigators work within the prison system, and as such are subject to enhanced vetting processes to gain clearance. Navigators undertake multi-vulnerability screening assessments of individuals and formulate plans for addressing any identified needs when they transition back into the community, working under the indirect supervision of a registered healthcare professional. The team support people from 12 weeks pre-release from custody up to 6 months post release.

Main duties of the job

The post holder will be responsible for working within a multi-disciplinary team, with the flexibility of working across Staffordshire, dependent on skill and service need. The Post holder with be part of a multi-disciplinary team, primarily completing screenings of individuals needs at different stages of their journey through the Criminal Justice System.

The post holder will use screening tools to identify health and social care needs. The post holder will assist individuals to navigate health and social care services to address identified needs, will be required to complete onward referrals where appropriate and provide short term intensive support to patients and their families.

The post holder will be required to provide high quality information to key decision makers in youth and criminal justice agencies, including the police, courts, probation and Youth Offending Teams (YOTs).
