Job summary
We are seeking an experienced, enthusiastic and motivated Registered Learning Disability Nurse (LD) to take on the role of LD Acute Liaison Specialist Nurse for people with learning disabilities within North Staffordshire
The service covers the geographical area of Stoke-on-Trent, where the post holder will be work as part of the Community Learning Disability Health Team (CLDHT). However, the post holder will spend a large amount of time working within Royal Stoke University Hospital (RSUH), working closely with acute teams and services.
The roles focus is raising the awareness of the health needs of people with a learning disability, in order to achieve the best health outcomes for our local population or those accessing acute services.
Influencing change is a fundamental part of the Acute Liaison Nurse role. Prioritizing clinical needs on a daily basis is also a skill essential for this role.
We are seeking a confidence person to work autonomously with strong advocacy skills and relationship building experience. Someone committed to not only support the improvement of care pathways but to also develop and deliver training and education packages to a diverse group including people with a learning disability, carers and professionals.
The successful candidate will work as part of a small Health Facilitation Team, with the full multi-disciplinary support of the CLDHT.
The service operates from 9-5 Monday - Friday, flexible working patterns can be discussed .
Main duties of the job
Be a member of the Community Learning Disability Health Team however work and be based primarily within the acute hospital setting, there may be a requirement to work with clients in a variety of other health / community settings.
Utilise advanced knowledge and skills to work with adults with complex needs to meet defined objectives, reduce health inequalities by supporting people with mild, moderate or severe learning disabilities to access mainstream healthcare.
Ensure health professionals gain knowledge and skills in being able to provide healthcare that is responsive to their individual needs.
Participate in the development and improvement of a learning disability services.
Participate in the development and provision of the LD Specialist Acute Liaison Nursing in North Staffordshire.
Participate in the planning, development and evaluation of Health Facilitation and Acute Liaison services within a designated area / team, co leading and contributing to relevant projects.
Will be required to educate staff including both students and qualified and unqualified team members on key LD agendas .
