Job summary

Community Hospital Alcohol Team is a community-based team working closely with UHNM to provide alcohol support to the community of Stoke on Trent for patients that attend emergency portals with harmful use of alcohol.

We reduce the risk associated with alcohol and promote change to improve the health and social welfare of patients. The community alcohol support worker (CHAT) will support the alcohol detox nurses within the team. This post is part time, 30 hours per week.

We offer a 7-day nurse led service and our hours are 8am - 4pm providing increased support as required.

We provide Psychosocial Interventions for up to 12 weeks from discharge at UHNM, this being harm reduction, relapse prevention and seamless referrals on to recovery services.

Main duties of the job

To support earlier discharge from hospital / admission avoidance for suitable patients with alcohol related problems.

Assist Service Users who are utilising multiple emergency services i.e. ambulance, A&E, IOU, Harplands, crisis teams.

Provide increased support for service users with co-existing complex physical health needs (e.g. chronic liver disease, end of life) that require a coordinated case management approach.

Deliver increased support for people known to be difficult to engage in services and who require an assertive management approach (e.g. require home visits, greater number of contacts to remain engaged).

To assist Registered Professionals in assessments and care planning/ implementation and evaluation of individualised care plans.

Work in partnership with service-users to assist them in identifying recovery planning that is meaningful to them and supports agreed health outcomes.

Regularly supports service-users to access a range of activities and services to maintain their independence and promote their health and wellbeing.

Regularly provides advice and guidance to Service-users promoting healthy activities and lifestyle choices.

Regularly supports Registered Practitioners and Service-users during activities to meet their physical substance misuse and mental wellbeing.
