Job summary

An exciting opportunity for a Band 3 Healthcare Support Worker for Child and Adolescent Health has arisen working alongside the ARRS Mental Health Team within Primary Care.

Band 3, £24,071 - £25,674 per annum, full time 37.5 hours per week.

Working in conjunction with a registered mental health professional and to support the delivery of a project to strengthen the liaison between North Staffs Combined Healthcare Children and Young Persons mental health services and Primary Care.

The role will support the review of CYP referred from primary care and assess where their needs can be best met - increasing access for mild to moderate mental health needs of CYP.

The post will support interventions with children, young people and families as well as some administration duties to support the team in the collation of performance data to support the project delivery.

You will work closely with the GP practices, senior mental health practitioners and STR workers, to identify children and young people who require early help and support with their mental health but may find it difficult to access the range of services currently available due to social circumstances or significant vulnerabilities.

  • The post-holder is responsible for the provision of planned person centred care working in partnership with children, young people (0-25yrs) and parents/carers.
  • The post-holder is responsible for working collaboratively with other Trust services to provide seamless care for service-users on defined care pathways
  • The post-holder is responsible for working in partnership with the Primary Care Network and with agencies external to the Trust supporting service-users access to services in the independent and non-statutory sector.

The post holder will undertake the following duties and responsibilities at all times demonstrating the competences assigned to the role detailed in the associated KSF outline:

1. Communicate with children, young people and families, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including Information Technology.

9. Under supervision responsible for contribution towards assessment and evaluation of planned recovery focused therapeutic activities with an identified group of children, young people and families.

10. Responsible for undertaking elements of a holistic assessment, under the direction of Registered Health Professional staff. Responsible for regularly working with children, young people and families to negotiate and review existing plans to address identified needs under the supervision of Registered Professionals.

11. Work in partnership with children, young people and families to assist them in identifying activities that are meaningful to them and that support agreed health outcomes.
