North Staffordshire Combined Healthcare's Liaison and Diversion Team are looking to recruit a full time Band 6 Liaison and Diversion Practitioner.
The Liaison and Diversion service is commissioned by NHS England in order to provide a Staffordshire wide service with the intention of early assessment, information sharing and referral onwards for people in contact with the Criminal Justice System. This service is based in criminal justice environments including police custody facilities and the magistrate's courts, with a view of identifying and supporting people to address a range of vulnerabilities including Mental health, learning disability, accommodation needs, financial needs, substance misuse and alcohol problems. The Liaison and Diversion practitioner's role is to undertake assessments of those who present within Criminal justice environments (police custody and court) to identify any vulnerabilities, ensure appropriate liaison and diversion opportunities are presented to key decision makers in the criminal justice pathway and make referrals into appropriate health and social care pathways to address any vulnerabilities identified.
Main duties of the job
The post holder will undertake and action appropriate assessments to identify and meet the needs of those they assess within police and other criminal justice settings.; ensure that practice remains abreast of new developments; develop and maintain standards and implement change in respect of this. The postholder will liaise with other professionals and agencies both statutory and non-statutory, provide a link between all agencies involved in the care and management of Liaison and Diversion service users. Forging and maintaining relationships with partnership agencies is key to all roles within the Liaison and Diversion service.
