Job summary

We are excited to announce that the Neuro Community Team are looking to recruit into our specialist Neuropsychiatry service.

The service based in Stoke on Trent, specialises in the assessment & support of adults who are experiencing complex mental health problems related to either an acquired or degenerative brain condition.

The team compromises of Neuropsychiatrists, CPN's, a Nurse Practitioner, Occupational therapists, STR workers & Neuro psychologists. We also promote and support a learning environment for students.

The team provides a unique service to clients with diagnoses such as Huntington's disease, Parkinson's disease, cognitive conditions & acquired brain injury. Working closely with these complex individuals, their carers & other care providers to help them receive through assessment & specialist support; allowing them to reach their optimum.

We are looking to recruit a band 5 CPN to join our community team, who can contribute to delivering an excellent service to our client group and support in the ongoing development of our unique & complex service.

The successful applicant will be required to in-reach into our in-patient service in order to support seamless care to our clients across community and in-patient services.

Experience in neuropsychiatry is desirable, however, specialist support & training will be provided to the successful candidate.

Main duties of the job

To work as part of an interdisciplinary team to diagnose, assess, plan and implement patient specific programmes of care/interventions

To undertake complex assessments, using your profession specific clinical knowledge and skills.

To work effectively across a variety of environments (e.g. ward, department, patients' homes etc.)

To use a comprehensive range of verbal and non-verbal communication tools to communicate effectively with patients, this will include patients who may have difficulties with understanding or communication.

To evaluate patient progress and alter treatment programmes as and when appropriate.

To be responsible for own time management and the prioritisation of referrals.

To supervise the practice of designated non-registered staff within the team, including nursing students.

To develop comprehensive discharge and contingency plans with relevant professionals to arrange on-going care.

To contribute to professional meetings, liaise closely both verbally and in writing with the multidisciplinary team, patient and carers across boundaries to ensure continuity and integration of patient's treatment.

To assume care co-ordinator responsibility for a caseload of patients and to ensure that each has a current, individualised and comprehensive care plan and risk assessment based upon assessed need.

To carry out all care and associated tasks for the caseload without direct supervision.

Supervise junior/unqualified staff in the provision of care.

To contribute to the day-to-day operation of the clinical area in respect of delegated administrative procedural tasks.

To support senior staff in the planning and implementing of new initiatives.

CLINICAL

Care Co-Ordinator Responsibilities

To act as care co-ordinator for a group of patients. Develop a positive working relationship and promote engagement with the patient and significant others (subject to the patients consent). To ensure that the patient is supported through all aspects of the care process and that care is planned according to individual needs identified through assessment. To liaise with other professionals and non-statutory agencies as appropriate regarding community care.

Ensure that the rights of the patient are upheld in accordance to relevant legislation (e.g. mental health act, human rights act). Provide necessary written information and support the patient in developing an understanding of this, if necessary referring to an independent advocate.

To be responsible for the overall written records, ensuring that treatment care plans are clear, current and concise. Ensuring that care is reviewed in responses to changes in presentation and that this is communicated both to the patient and clinical team. Ensure that assessments (including risk assessments) are completed in a timely manner and review dates are is identified. Maintain written standards in accordance with statutory guidelines.

Carry out care and associated tasks for patients within the clinical area. Ensure that changes in presentation are reported where necessary acted upon. Ensure prompt and efficient communication with other team members regarding any changes in condition or plans of care.

To deliver specific therapeutic interventions (e.g. psychological interventions, anxiety management, observation, counselling etc) according to the clinical area, patient need and personal levels of skill. These may be delivered on either a group of an individual basis. The post holder may deliver these directly or act in a supportive co/facilitate role to other team members.

Attend and when appropriate lead clinical meetings (e.g. Ward rounds, MHRT, CPA/Care Co-ordination meetings). Ensure that discussion is accurately documented, that verbal reporting is accurate and concise and that required actions are clearly identified and pursued. Ensure that outcomes are accurately reported to significant others.

PROFESSIONAL AND STATUTORY OBLIGATIONS

Ensure personal practice and that of those supervised adheres to relevant Codes of Conduct, legislation and policies and procedures. (See appendix for examples). To be aware of the implications of these, particularly those concerning patients rights, and take any necessary action in the event of non-compliance.

Take responsibility for personal and professional development in accordance with statutory guidance, attending statutory and mandatory training and other pertinent training as agreed with the clinical manager through the personal review process. Maintain registration and personal professional portfolio.

Ensure that professional knowledge remains abreast of current trends and recent evidence in the field of activity

MANAGERIAL

To report to the person in charge/clinical manager any matters that are likely to affect the provision of patient care within the area.

Monitor the physical environment with due regard for standards in relation to hygiene and safety. Maintain a safe and orderly environment, identify potential hazards and where possible take appropriate action, if necessary reporting to the person in charge and/or liaise with support services staff. Promote the active involvement of the patient group (where appropriate) in maintaining a clean and orderly environment.

Ensure that resources are utilised effectively and stored correctly. Participate in auditing or requisitioning procedures acting on any deficiencies as appropriate.

To occasionally take charge of the clinical area in the absence of a senior manager. (An average of once per week) To have responsibility for the management of the clinical area, the prioritisation of tasks and supervision of junior staff during this time.

To undertake specific management or administrative tasks during a shift as delegated by the person in charge and under their supervision.

To supervise and monitor the conduct of junior staff providing appropriate guidance, support and feedback, report any performance related issues to the person in charge.
