Job summary

If so, a fantastic opportunity has arisen for an for an experienced Learning Disability Clinician from any registered professional group to join the Intensive Support Team and take on the role as the Team Deputy.

North Staffordshire Combined Healthcare NHS Trust provides community based accessible health services to people with a learning disability who may require additional support to that available from mainstream services. The post holder will support the Team Manager to lead the team in the rapid response, intensive assessment, treatment and intervention for individuals with highly complex needs who may be at risk of hospital admission and/or placement breakdown. This providing a realistic alternative to hospital admission by providing intensive support to service users within their own community settings at the time they most need it in order to avoid further deterioration. The deputy team lead is a lead role in improving the health and wellbeing of adults with learning disabilities through an intensive support model in a time limited framework.

The service is wholly committed to the use of Person Centred Planning approaches and affording adults with learning disabilities opportunities to maximize full citizenship.

Main duties of the job

Provide Deputy Team Lead support for Intensive Support Team (IST).

The IST it is commissioned for 7 days per week between the hours of 8am and 8pm.

Participate in the development of an evolving learning disability service.

Have specialist skills to assess service users' needs and analyse and formulate treatment plans within a multidisciplinary framework, making recommendations for theirs and others practice.

Ensure that the assessment and treatment of an individual occurs in a setting that is the least restrictive and risk implications are assessed.

Work in collaboration and promote partnership working with other statutory, non within statutory and voluntary organisations.

Provide professional advice to the integrated teams on learning disability

Develop complimentary roles as agreed with the Caseload Manager to meet objectives for improving the health for adults with learning disabilities as set out in the Department of Health "Valuing People - A New Strategy for Learning Disability for the 21st Century", "Valuing People Now" and ''Building the Right Support'' (NHSE 2015) These roles will incorporate health promotion, facilitation, treatment and service development.

In conjunction with the Team Lead, to take responsibility for setting, maintaining and evaluating standards of care.

Initiating and participating in team audit processes as requested by the Team Lead.
