This post is within our substance misuse team and the role will involve working as a substance misuse staff nurse within a multidisciplinary team, this includes primary and secondary physical and mental health services and the wider prison establishment. The clinical team work from our substance misuse recovery wing and are committed to delivering a respectful, caring and compassionate service to this complex client group. Your role will involve assessment, care-planning and risk management alongside a range of both clinical and psychosocial interventions. We are working towards developing a co-morbid mental health and substance misuse service.

Main duties of the job
Under the supervision and direct management of the Integrated Substance Misuse and Mental Health Manager, you will contribute to the provision of high quality substance misuse services which provides recovery focused clinical treatment for the prison population within Stoke Heath HMP. To work within a strengths-based, recovery-orientated, change and outcomes focused approach which promotes service users as responsible co-producers of health, well-being and recovery. To work collaboratively with other services to provide interventions to support and motivate service users and meet their needs through all stages of their individual recovery journey
To ensure that practice remains abreast of new developments and to develop and maintain standards and implement change in respect of this.
To liaise with other professionals and agencies both statutory and non-statutory and to provide a link between the prison and community.

North Staffordshire Combined Healthcare Trust, as part of Stoke Heath Integrated Care, provide high quality and innovative substance misuse services to the population of Stoke Heath Prison. Stoke Heath is a closed category C male and young adult resettlement prison. It is situated near to Market Drayton, Shropshire.
We are a small dynamic and passionate team who strive to provide the best evidenced based care possible. We have an active clinical caseload of 100 patients and we are working towards the development of a Co-Morbid Mental Health and Substance Misuse Service. We provide great developmental opportunities for staff.
Our service operates 0730-1530hrs over a 7 day week.

Job responsibilities
1.1. To act as named nurse for an identified group of service users within the clinical caseload providing recovery focused, evidence based clinical practice to facilitate effective treatment decisions.
1.2. Engaging with the prisoners who have substance misuse problems, to address their individual issues working alongside the medical and psychosocial therapies teams to improve health and social functioning.
1.3. Initiate service users safely onto clinical treatment regimens ensuring that clinical protocols are constantly reviewed to promote best evidenced clinical practice linking to National Guidelines and Trust policies and procedures.
1.4. Undertake assessments and having the responsibility for documenting the plans of care plans ensuring they remain current and that standards of record keeping are adhered to. To take an active role in auditing and maintaining standards of record keeping for this group of prisoners.
1.5. Develop and provide care planning that supports health education linking to harm minimisation, blood borne viruses and overdose prevention.
1.6. To contribute to the seamless transition of the service users clinical needs and ensuring that care planning supports liaison and referrals to other departments involved with the service users broader
Care planning including: - Recovery Workers, Health Care, Offender Management, psychosocial Interventions, DIP and other community based services who will take ongoing governance responsibility for prescribing requirements upon release.
1.7. To have responsibility for seeking support and advice from the Senior Nurse in respect of issues and difficulties with individual clinical care. To attend regular decision making meetings with the clinical manager in order to address caseload management issues.
To administer and manage medicines according to policy and local protocol. To assess service user response and provide advice and education to the service user in respect of prescribed medicines.
1.8. To provide support to other service users within the clinical team when the named nurse / worker is unavailable. E.g. periods of annual leave sickness etc.
