We are seeking an experienced, enthusiastic and motivated RNLD to take on the role of Learning Disabilities Specialist Nurse with Acute Services
The service covers the geographical area of Stoke-on-Trent, where the post holder will be work as part of the Community Learning Disability Health Team (CLDHT). The post holder will spend a large amount of their time working in close partnership with the Royal Stoke University Hospital (RSUH).
Raising the awareness of the health needs of people with a learning disability, influencing change is a fundamental part of the Learning Disabilities Specialist Nurse role.
The role will include supporting acute services to meet the health requirements of people with complex needs.
We are seeking someone with the confidence to work autonomously, the experience to not only support the improvement of care pathways but to also develop and deliver training and education packages to a diverse group including people with a learning disability, carers and professionals.
The successful candidate will work as part of a small Health Facilitation Team, under the clinical guidance of a Lead Health Facilitation Nurse and with the full multi-disciplinary support of the CLDHT.
The service operates from 9-5 Monday - Friday, flexible working patterns can be discussedand this role attracts all of the usual benefits that would be afforded from being part of the NHS.
Main duties of the job
North Staffordshire Combined Healthcare NHS Trust aims to provide accessible, non-stigmatising local services that contribute to the assessment, intervention, treatment, rehabilitation and enablement of people with learning disabilities, which promotes the prevention, early detection and treatment of health related issues for adults with learning disabilities in the community.
The Community Learning Disabilities Health Team (CLDHT) provides health facilitation by supporting, facilitating and advocating ensuring that people with a learning disability gain full access to the health care they need, whether from a primary, acute, secondary or other providers. The service provides support by raising awareness through education and supporting professionals to identify their learning disability patients, and making any reasonable adjustments necessary as well as offering advice and guidance to professionals when working with someone with a learning disability.
The service is wholly committed to the use of Person Centred Planning approaches and affording adults with learning disabilities opportunities to maximize full citizenship, choice and control. The Heath Facilitation Nurse has a lead role in promoting, enhancing and maintaining the health and wellbeing of adults with learning disabilities.

Develop complimentary roles as agreed with the Team Manager to meet objectives for improving the health for adults with learning disabilities as set out in the Department of Health White Paper "Valuing People - A New Strategy for Learning Disability for the 21st Century" and "Valuing People Now and Building the Right Support (2015). These roles will incorporate health promotion, facilitation, treatment and service development.
