Job summary

We are seeking a Band 7 Independent Prescribing Lead Nurse to take up a valued post within our Learning Disability Service.

We have an exciting opportunity for an experienced, enthusiastic and motivated RNLD with additional prescribing qualifications to join our community Team.

The service covers the geographical area of Stoke-on-Trent, where you will be expected to work in partnership with our Psychiatry team, GP Practices and learning disability teams to ensure that the health needs of people with learning disabilities are being met effectively. A key part of the role will be to influence pathways of care to ensure 'reasonable adjustments' are implemented to reduce health inequalities and to support the STOMP agenda. Holding a caseload ,running clinics and reviewing client's treatment regimens will form a large aspect of the role.

You will hold a senior lead nursing position within the CLDHT and as a result will be required to provide supportive clinical leadership, coaching and supervision within the wider nursing team.

You will be expected to lead on the continuous improvement of clinical nursing practice within the team and contribute to the development of junior nurses, to inspire, improve performance and increase self-esteem within nursing; as well as acting as an expert resource in relation to clinical nursing within learning disabilities.

The service operates from 9-5 Monday - Friday though flexible working can be negotiated.

Main duties of the job

Under the supervision and direct management of the Team Manager you will:

Be a member of the Community Learning Disability Health Team and participate in the development of an evolving learning disability service.

Provide nursing advice, guidance and leadership to the integrated teams on learning disability nursing issues.

Contribute to the provision of high quality community learning disability services for the population of a defined area within North Staffordshire.

Develop complimentary roles as agreed with the Team Manager to meet objectives for improving the health for adults with learning disabilities as set out in the Department of Health "Valuing People - A New Strategy for Learning Disability for the 21st Century", "Valuing People Now" and Building the Right Support (NHSE 2015) These roles will incorporate health promotion, facilitation, treatment and service development.

Work within the learning disability clinical pathways to provide an outcomes focused approach which promotes service users as responsible co-producers of health, well-being and recovery.

Work collaboratively with other services to provide interventions to support and motivate service users and meet their needs through all stages of their clinical / care pathway.

Contribute to the medical prescribing provision within the community learning disability team through supportive dedicated Non-medical prescribing. This will include supporting the delivery of a number of Consultant Psychiatry led clinics and holding an independent caseload within the North Staffordshire area.

Act as lead professional / care coordinator for a specific group of clients and provide care under the supervision of the Team Manager. To deliver specific psychosocial interventions to this group of clients and liaise with other professionals regarding progress and outcomes.

In consonance with the responsibilities of this role, lead and deliver specific clinical projects and assignments, as directed by the Team Manager and Directorate Managers.

Ensure that practice remains abreast of new developments and to develop and maintain standards and implement change in respect of this.

To liaise with other professionals and agencies both statutory and non- statutory as necessary.
