To clinically support with undertaking delegated duties as directed by the SDEC team when managing a cohort of patients who are under the clinical responsibility of the SDEC Team.
To support and work alongside the SDEC team with treating and providing basic care needs of patients that present within the SDEC Unit in line with local protocols and SDEC Unit pathways.
Main duties of the job
To clinically support with undertaking delegated duties as directed by the SDEC team whenmanaging a cohort of patients who are under the clinical responsibility of the SDEC Team.
To support and work alongside the SDEC team with treating and providing basic care needs of patients that present within the SDEC Unit in line with local protocols and SDEC Unit pathways.
To work closely with staff in Emergency Department, Urgent Care Team, General Practitioners and Capacity team whilst maintaining an efficient, effective, safe delivery of patient care within the Ambulatory Care Unit.
Support the flow of patients from the Emergency Department to Ambulatory.
Emergency Care in line with Ambulatory care Pathways.
Reporting any changes of patient's conditions to one of the SDEC's clinicians or Nursing staff.
Maintain accurate appointment, filling and coding records as required.
Support the Lead Receptionist and admin with any administrative tasks that may be required.

To perform a range of nursing skills as per trust policy including venepuncture, cannulation, recording ECGs and clinical observations.
- To participate in all elements of direct patient care including hydration, nutrition, continence, pressure area care and harm prevention.
- To ensure all trust documentation pertaining to patient care and departmental safety are completed to a high standard.
To promote good working relationships within the nursing teams, providing appropriate advice and support.
To escalate deteriorating patients in a timely manner and taking responsibility for supporting the team in such situations.
- Perform cannulation and venepuncture as directed by nursing team.
- Perform vital signs monitoring, ECG recording and Blood Glucose monitoring, reporting abnormal results to registered nursing staff as appropriate.
- Lead on managing the clerical aspects of Ambulatory Clinics, including booking and inputting outcomes/appointments onto Lorenzo and other appointment systems.
Lead on coding Ambulatory Care appointments.
Requesting and filing notes of Ambulatory Care Patient
Working within set guidelines and Trust policies recognising one's own abilities and limitations.
