Main duties of the job

The post holder has the responsibility of managing/supporting the timely admissions/discharges through the medical pathways. The role will require interface with all departments to review and constantly assess medical capacity and flow whilst ensuring excellent positive patient experience for all. This includes being well informed of any impacts that may result due to stagnation in any part of the medical pathway from admission, to ward and be able to resolve these flows to ensure the patient receives the right care, in the right place, by the right person.

The post holder will maximise the use of inpatient beds enabling local and national NHS targets to be met using knowledge, skills, experience, influence, negotiation and advanced communication skills to identify and resolve issues that could impact on patient pathways.

The post holder will lead on improving discharge processes within the departmental areas and be a key driver in embedding changes in practice which is evidenced by data collection and audit.

The discharge manager will support the Ward/General Managers in the daily review of medical capacity. The role will be advisory to encourage ward managers to inform ward level ownership of admissions and discharges, whilst informing site teams of progress and updates throughout the day.

Job description

Job responsibilities

  • To proactively manage patient flow to ensure achievement of work streams within medicine.
  • To develop optimal bed utilisation and ensure the most appropriate use of Trust beds for medical patients. Utilise clinical skills /knowledge/ judgement to ensure beds are being used in the most effective way; have problem solving and negotiating skills to deal with difficult situations.
  • To take prompt corrective action and/or escalate any patient flow or clinical site issues within medicine.
  • To work collaboratively with multi-professional teams across the Trust to proactively facilitate the safe transfer of patients from Emergency Department, all inpatient wards.
  • To establish effective working relationships with all nursing, and support staff, medical staff, general managers, site teams, community teams and the transport services. Work and liaise with the infection control team particularly where outbreaks or infection control guidance is required
  • To prioritise own workload, act promptly and effectively to meet the regular unpredictable situations that may occur within this role
  • To deliver a high level of visibility within the Emergency Department, Acute Medical Unit, inpatient wards in maintaining the emergency care pathway and
  • To ensure effective & appropriate use of beds and all resources, maintaining clinical quality & safety, in accordance with Trust policies & procedures and national quality
  • To participate in the operational bed meetings to provide an overview situation report on the bed capacity and flow within surgery with the ability to interpret any risks and contribute to mitigations.
  • To provide and maintain accurate live information on bed availability by providing accurate information through the databases on levels of emergency demand, patient outlier numbers, theatre capacity and bed occupancy and planned admissions for the day and subsequent days.
  • Liaise with complex discharge team to promote the most effective movement of patients within a timely
  • To participate in service improvement, audit and data collection with particular focus on embedding the use of the SAFER patient flow bundle within the ward
  • To assist in ensuring newly appointed doctors are aware of their role in the admission process and bed management
  • To have an awareness of quality issues regarding the inpatient journey within the Trust
  • To monitor and report any concerns to the appropriate lead matron on standards of care observed during clinical working and ensure appropriate follow up to said
  • Take responsibility for maintaining self -development and seeking educational opportunities as appropriate, to support revalidation and registration, and ensure Professional Code of Conduct is upheld
