Job Title: Anticoagulation Clinical Nurse Specialist

Hours per week: 37.5 hours per week

Perm/Fixed Term: Permanent

Salary: £35,392-£42,618 per annum

Main duties of the job

Working as part of a multi-professional team to deliver enhanced clinical practice to a specific client group and participate in education, audit and application of evidence-based practice within the specialty.

Specialist Practice

  • Manage daily anticoagulation nurse led clinics
  • Receive direct referrals from General Practitioners and Consultants
  • Manage and co-ordinate all investigations and treatment for DVT patients from referral to diagnosis according to the DVT protocol
  • Participate in approved Patient Group directions by the Trust appropriate to our sphere of practice, to maintain safe anticoagulation for patients.
  • Co-ordinate patient discharge liaising with all members of the multidisciplinary team to ensure that safe delivery of anticoagulation continues as an outpatient
