To work autonomously within Heart Failure Community Service to provide administrative support for the Community Heart Failure Nurses such as booking patients on to clinic and communicating between both SWFT and GEH.

Main duties of the job
Skills to manage own caseload which will be signposted by Cardiology Nursing Staff.
Communicating any relevant information to Registered nurses regarding their patients and the wider multidisciplinary team.
To provide welfare home visits to patients and to carry out blood taking, ECG and basic clinical assessments under the supervision of the Heart Failure Nurses.
Facilitating a seamless transition on discharge to the Coventry Heart Failure community service and SWFT Community and liaising with the Heart Failure GE@Home service.

MAIN DUTIES AND RESPONSIBILITIES
- Responsible for own actions, working within agreed guidelines, recognising the need to refer to a trained member of the team as appropriate.
- To be involved in the induction, training, supervision and provide mentorship to new starters, students and practice nurses.
- Responsible for delivery and modification of patient care in line with protocols, ensuring all care is accurately documented.
- Demonstrate an in-depth knowledge of service interdependencies within primary and secondary care, including eligibility and referral criteria: for example, Cardiac Rehabilitation, FAB (Myton), palliative care, HF nursing teams in North/South Warwickshire and Coventry/Rugby, Community nursing and Age UK.
- To effectively communicate appropriate information regarding a patients condition and any other required information to the wider multidisciplinary team.
- To work without direct supervision, but seeking guidance from a qualified member of the team as required. Duties include:
- Initial assessment of patients, delivery of appropriate level education on self -management and common HF medications utilised in its management
- Taking blood and basic investigations if it facilitates discharge
- Assessing the performance of activities of daily living
- Liaising with SWFT community and community HF nurses, social services, Intermediate Care and additional community services to arrange additional support for patients in order to facilitate discharge and readmission.
- Identifying and ordering of appropriate equipment.
- Ensuring IT systems are updated.
- Booking patient appointments within the HF clinics on Lorenzo and discharging when appropriate. General administrative duties to assist with the smooth running of the clinics.
- Utilise IT systems to secure accurate, timely patient data. To enter HF team data on the specialist database and maintain equipment provision, for the purposes of audit, within data protection and information governance guidelines.
- Accurately record all care given, education given, information HF packs and assessments made.
- Communicate all relevant information to other staff who are working with you.
- Manage expected clinical workload requiring unpredictable levels of physical effort according to patient dependency/clinical need, reporting to a senior member of the team where appropriate, e.g. In an unexpected event.
- Support staff and patients exposed to difficult situations arising in the clinical area e.g. after receipt of bad news or following an unexpected event.
- Promote and monitor adherence to Health and Safety and Trust policy designed to protect healthcare staff and service users from known hazards.
