Main duties of the job

The post holder will work towards:

Support patients/families during the diagnostic process, treatment phase and terminal stages

Facilitate breaking bad news sessions between clinicians and patients

Conduct a full assessment of a patient/family's needs including holistic, physical, psychological, social, spiritual aspects of care and formulate a plan of care to meet those needs

Provide highly complex emotional support, advice on symptom control, information regarding the disease to patients, relatives and health professionals.

Act as Key Worker between patients and families and the lung cancer team.

Make decisions on referrals to other health professionals to facilitate optimum care.

Co-ordinate care with other professionals, in acute Trust or community so that care is organised efficiently, and seamless care is maintained.

Act as a resource to those involved in lung cancer care, providing knowledge on the physical, psychological, and social impact of the disease.

Maintain own patient case notes and provide written documentation in medical and/or nursing notes.

Attend and participate the weekly Lung MDT meeting, contribute to the discussion, act as an advocate. Work with Lead Lung CNS to ensure decisions are effectively coordinated and communicated to other health professionals or to the patient.

Provide up-to-date relevant written information to patients and families regarding lung cancer, its diagnosis, treatment, and long-term care.

RESEARCH, DEVELOPMENT AND AUDIT

The post holder will:

1.Be involved in local/national research proposals to develop lung cancer services nationally.

2.Disseminate research findings locally and, if required, at national level to produce an evidence base for lung cancer nursing on behalf of the Trust and the Cancer Network.

3.To be aware of and disseminate recent advances in lung cancer.

4.Participate in the collection of statistical information to inform the operation activities of the lung cancer service, specifically in relation to the National Lung cancer Audit (NLCA), working with Cancer Data Manager as required

5.Establish an accurate record keeping system for personal use and contribute to the development of care and treatment planning by recording in medical and nursing notes.

6.Participate in the developments of evidence-based policies, guidelines, and protocols to improve standards of lung cancer care.

8.Contribute to the National and Regional development of lung cancer services through activities within West Midlands cancer alliance Lung EAG and the National Lung Cancer Nurses Forum.
