An exciting opportunity has arisen for a Registered Nurse with evidence of appropriate post registration experience and a special interest in the care and management of people with Diabetes Mellitus to join the Diabetes CNS Team at George Eliot Hospital NHS Trust, Nuneaton.

Main duties of the job

The post holder will join a team of Clinical Nurse Specialists and other members of the multidisciplinary diabetes care team, and support the education and management of people with diabetes both within inpatients and outpatients.

Providing evidence based and high quality diabetes care and education

Providing specialist Diabetes advice to patients, carers/relatives, and the multidisciplinary team.

Contributing to the organisation and delivery of ongoing education and training for other healthcare professionals.

Job responsibilities

KEY RESPONSIBILITIES

  • Clinical

Provides high quality nursing care, education and support to patients with diabetes and their carers within the acute and community setting without supervision.

Facilitate patients to take an active role in their condition by understanding the principles of treatment, recognition of adverse changes and appropriate action to be taken.

Gives advice, education and information to both newly diagnosed and long-standing patients with diabetes as inpatient and outpatient service users.

Liaises with nursing staff in adult clinical areas regarding the care of patients with diabetes. Visits patients in order to assess nursing needs and educational requirements where appropriate.

Liaises with community nurses and multi-disciplinary team to ensure continuity of patient care.

Develops and delivers clinical education for both nursing staff and patients that is evidence based and up to date.

Acts as a resource for link nurses, staff and patients for diabetes care.

With the consent of the Consultant Diabetologist adjust insulin therapy in line with agreed protocols.

Participates in audit and research

Undertakes nurse-led clinics,

Works within the NMC Guidelines and Trust Policies.

  1. Managerial

Deputise for the Lead Nurse in Diabetes in his/her absence.

Ensure the Diabetes Clinics are well organised and efficiently run.

Operate nurse-led clinics without direct supervision.

Assist with the management of department resources in an efficient and effective manner.

  1. Education and Training

Facilitate/organise diabetes education sessions for patients, carers and staff in all aspects of diabetes care in collaboration with the Lead Nurse in Diabetes.

Assist with the appraisal process and facilitation of professional development of staff within the Specialist Team.

In collaboration with the Lead Nurse in Diabetes, facilitate the development of nursing practices based on up to date information and research.

  1. Professional

To act as an ambassador for the clinical environment and the Trust where appropriate by promoting a professional image of nursing.

Ensure current registration with NMC is maintained.
