Community Support Worker - Integrated Referral Hub

Job summary

We have an exciting opportunity to come and join our expanding Community Integrated Referral Hub team. .

This role is an integral part of the CIRH team, as a Community Support Worker, you will be the first responder to non injured patients who have fallen in their own homes. You will provide them with emotional and physical support to regain their balance.

You will have autonomy with the patient while in their home by way of assessing the situation and deciding if onwards referrals can and need to be made in order to support the individual to remain in their home for the better.

When you're not in the office, you will be expected to support as a call handler, taking the calls form the community and processing them quickly but precisely to ensure there is no delay for the patient.

As this is a new and exciting role, there will be plenty of opportunity to grow and develop yourself and your skill set.

As we are a 7 day service, the hours will be worked across those 7 days on a rota basis. You will be integrated within the team to meet the demands of the business.

Hours and days can be flexible to support the candidate but also needs to align with business needs. When working 1:4 weekends, you will be required to work 2 x 12 hour shifts due to the needs of the business but this can be discussed at interview.

Main duties of the job

Once a call has been made to our Community referral hub (CRH) you will be required to attend the patients home within a 45 minute target time.

  • Assess the patient to ensure that they are not injured and it is safe for you to assist
  • Support the patient in regaining an upright position, with the support of a multitude of equipment
  • Support the patient with any basic care requirements
  • Complete manual handling as appropriate for the scenario and in line with manual handling guidelines
  • Complete the evaluation form using our EMIS system for the patient
  • Complete any onwards referrals for the patient
  • Add any follow up calls & complete when neccessary
  • Be part of the whole CIRH (Community Integrated Referral Hub) Team
  • Take phone calls from community services, patients and other HCP's and ASC professionals and complete the referrals on the EMIS system
  • Being able to multi-task and using an array of different systems
  • Using a computer will be a big part of your role
  • Understanding that you are part of a much wider team
  • Being adaptive and moving quickly with new processes that are implemented
  • Having a positive and a can do attitude, especially when face to face with the patients.
