Job summary
An exciting opportunity has arisen for a crucial admin role as a Pathway Navigator for patients on Hepatology Cancer Surveillance and will be the central point of contact for the patients on HCC surveillance; helping to ensure the individual needs of the patient are supported and that care is built around those needs.
As a Hepatology Pathway Navigator, you will work closely with the Hepatology CNS team and Consultants, supporting the coordination and monitoring of surveillance pathways for patients at risk of developing Hepatocellular Carcinoma. This includes ensuring timely patient engagement in surveillance protocols, tracking key milestones in patient care, and maintaining surveillance databases.
As the central point of contact you will be involved with maintaining both effective communication with and on behalf of the patient, signposting to other resources and information as defined by patient need and liaising with staff and services to ensure tests and appointments are scheduled.
Main duties of the job
- Contribute to the implementation and maintenance of a liver cirrhosis database to enable the hepatology service to continue to deliver exceptional patient care and meet minimal national standards, as outlined in the "NHS England Hepatocellular carcinoma (HCC) surveillance: minimum standards" guidelines.
- Developing, monitoring, and maintaining a database of those on Hepatocellular Carcinoma surveillance
- Support the patient through the journey from identification and referral to conclusion of their intervention, thus eliminating barriers to care.
- Work closely with the wider gastroenterology MDT to ensure a patient focussed service and will keep accurate patient and activity records to facilitate reporting back to Commissioners regarding performance and service success.
- To evaluate surveillance data, to ensure the hepatology service meets national standards of care and provide regular data submissions to NHS England and the West Midlands Cancer Alliance.
- Ensure the trust meets national minimum standards of Hepatocellular Carcinoma (HCC) surveillance as outlined in the NHS England Hepatocellular Carcinoma (HCC) surveillance: Working on your own initiative with minimum direct supervision for the tracking of all patients diagnosed with liver cancer or suspected of having liver cancer, in order to ensure a timely diagnostic and treatment process and accurate entry of clinical data.
