We are looking for a new member to join the team who is patient focused and is keen to be involved in a dynamic environment where there is a constant drive for quality improvement to positively impact the outcomes of our patients. This is an exciting opportunity to be a part of the Integrated Discharge Team within WVT as a Band 4 Discharge Co-ordinator

We are looking for motivated and compassionate staff to focus on the patient discharge planning process supporting the safe and effective discharge for patients.

You need to be organised, able to prioritise workloads, use your own initiative and communicate clearly and concisely with a range of people. The role will involve working closely with ward staff, twice daily ward MDT meetings using the ethos of 'Why not Home, Why not Today' and communicating with patients, relatives and other community services to provide a proactive and continuous process of discharge planning, avoiding delays and thereby improving patient experience.

12 month Secondment opportunities considered.

Main duties of the job

In this vital role, you will:

  • Work in partnership with other members of the Integrated Discharge Team (IDT), Ward Multi-Disciplinary Team (MDT) and Community partners, to support decisions with regards to discharge planning
  • Represent the IDT at ward daily board rounds
  • Provide effective & timely communication of discharge actions
  • Be expected to undertake a range of duties without direct supervision but will be required to report back on those delegated duties to the ward team and case manager
  • Leasing with patients and families regarding discharge

We offer an induction period upon joining the team to introduce you to all the community services in Health and Social care who support discharge planning. We also offer a range of training opportunities to support the development of the right candidate.

  • NVQ/Diploma Level 4 or Foundation Degree Qualification in Health/Social Care or extensive experience within Hospital Discharge.
  • Completion of Care Certificate.
  • Good level of education, including English and Maths at GCSE level or equivalent

Skills, Knowledge & Abilities

Essential

  • Knowledge of discharge processes and the role of urgent care.
  • Computer literacy skills
  • Health and safety risk awareness
  • Able to work autonomously and use initiative, tact and sensitivity when working with patients with complex needs - seeking advice when needed
  • Ability to work unsupervised and prioritise own workload in a pressurised environment

Experience

  • Experience of working in a healthcare setting, and an understanding of complex discharge
  • Experience of working as part of a multi-disciplinary team
  • Evidence of commitment to learning (through portfolio)
