Job summary

The post holder will work primarily as part of the designated Hospital at Home nursing team as a Community Staff Nurse within the community setting supporting the team as well as therapy teams and ACPs to manage the effective delivery of services.

The Post holder will support the Hospital at Home service in providing professional nursing care to patients requiring a variety of nursing interventions, e.g. wound care, palliative care, IV antibiotics etc.

The post holder will be responsible for ensuring there is a comprehensive nursing assessment, risk screening and the development and evaluation of nursing programmes of care provided for the patients they provide care for in line with local and national guidelines.

The post holder will liaise with and refer to other health and social care colleagues, to promote the health and wellbeing and to rehabilitate people following illness or disability or those living with a long term condition.

The post holder will be expected to work flexible hours across a 7 day (pro rota) and via a shift pattern, to meet the needs of the service.

Main duties of the job

To work in a clinical nursing capacity within a community setting, primarily within a person's home.

To identify and record client's individual nursing needs through the framework of a single assessment.

To undertake necessary risk assessments/screening prior to implementing assessment and / or intervention.

Undertake nursing assessments in line with local guidelines.

To monitor progress, follow-up and evaluate nursing interventions with the client.

Empower users and families to participate in the management of their illness or disability by supporting an ethos of health promotion and self-management for all patients.

To work as part of the Locality Teams, liaising appropriately with all involved (e.g. District Nurses, Social workers, GPs, etc.) to ensure high quality of care is delivered to each individual.

To treat all people (staff and patients) with dignity and respect, and ensure confidentiality guidelines and information governance principles are adhered to at all times.

  • Registered Nurse Level 1 (Adult)
  • Current NMC Registration
  • Educated to relevant degree level or equivalent
  • Evidence of ongoing professional development
  • Mentor and assessing qualification (For newly qualified staff this is expected within 12 month of commencement)

Skills, Knowledge & Abilities

Essential

  • Demonstrate ability to teach others
  • IT literate (including Microsoft office) and willing to learn new systems
  • Knowledge of person-centred care and outcome based care planning
  • Knowledge of quality improvement processes including audit, risk management
