Job summary

An exciting opportunity has arisen for a Diabetes Specialist Nurse to join our dynamic diabetes specialist nursing team in Herefordshire.

If you do not have previous experience working in diabetes nursing, but are interested in developing a rewarding career in diabetes specialist nursing we can offer a band 5/6 development role with a tailored induction plan and dedicated time and support to achieve TREND-UK aligned diabetes specialist nurse competencies.

The successful candidate will work 28h/week as part of the multidisciplinary team across primary and secondary care with the aim of:

  • Providing and supporting high quality holistic individualised diabetes care to patients with complex diabetes management needs, their families and/or carers.
  • Preventing hospital admission
  • Promoting a greater understanding of diabetes and empowering patients, their families and carers and other agencies and professionals involved with the patient through education, support and counselling.
  • Nurturing collaboration and shared decision making

Alongside a senior diabetes specialist nurse, you will be responsible for supporting a caseload of patients with complex diabetes management needs, using evidence based practice and patient centred goals to assess, plan, implement and evaluate interventions.

Main duties of the job

  • Working as part of a wider Multi-Disciplinary Team, you will be involved in the assessment, review and complex care planning, employing the use of diabetes technologies, where appropriate, for people with complex diabetes management needs, in order to provide seamless, integrated care.
  • Representing diabetes specialist nursing team and attending multidisciplinary meetings and diabetes clinics in hospital and the community
  • Conducting patient reviews in a wide range of settings, depending on service and clinical needs.
  • Delivering structured education to people with diabetes, their families and carers/other agencies.

  • Completed/completing relevant degree
  • Diabetes qualification: Post graduate diploma in diabetes related field or equivalent experience, or equivalent experience or willing to work towards

  • NMC Independent Nurse Prescriber

Skills, Knowledge & Abilities

  • Awareness of local and national policy and initiatives in diabetes care
  • Confidence of intensifying diabetes treatment regimens to include advice on initiating and titrating insulin and GLP-1 medications as well as initiating and reviewing of Continuous Glucose Monitoring (CGM), or willing to learn
  • Excellent communication skills
