Therapy Assistant Practitioner - Community Stroke Team

Wye Valley NHS Trust

The closing date is 10 July 2024

Job summary

We are looking to recruit Band 4 Therapy Assistant Practitioners to our Community Stroke Service.

This is a part time role.

We will also consider a developmental band 3 post, with progression to band 4 following completion of competency training with occupational therapy and physiotherapy over 18 months.

You will be working within the community stroke team but also with hospital therapists, home care staff, and of course stroke survivors and their families. You will have a crucial role ensuring communication between these different teams is excellent and that the voice of the stroke survivor is heard by those providing a service to them.

You will also be providing rehabilitation in the community and patients homes including exercise, daily living assessments, cognitive rehabilitation and equipment provision.

In addition to joint working, you will also be signposting to sources of education and information and identifying training needs as they arise. This will involve signposting, collating information, liaising with other professionals and delivering some training sessions.

You will be supported in this role by the Community Stroke service and receive supervision and support from them.

Main duties of the job

To work closely with and advocate for stroke survivors on their discharge from hospital and throughout their rehabilitation with the Community Stroke Service.

Working as part of the therapy and wider multidisciplinary team, Therapy assistant practitioners use enhanced skills in a specialised area to provide direct high quality and effective therapeutic care and support to patients including those with more complex needs. Within the scope of the role, the post holder will independently (unsupervised) plan and deliver individual or team tasks where the understanding of role boundaries and the importance of supervision and carrying out appropriately delegated tasks and plans is essential.

This role requires understanding, and factual and theoretical knowledge of the principles of stroke rehabilitation. Although work is guided by standard operating procedures, protocols or systems of work, the post holder must to able make judgements, plan activities, contribute to service development and demonstrate self-development. The post holder may be responsible for supervision of others and delegate appropriate tasks to other non-registered staff.

The post holder will be required to complete the relevant competencies for the post and participate in 7 day working.

Candidates will also need to be able to fulfill the travel requirements of the post.
