Job summary
An exciting opportunity has arisen to join our Cardiac Rehabilitation Team providing comprehensive Cardiac Rehab Groups around Herefordshire and a supported telephone home based program?
Our team deliveries Cardiac Rehabilitation (exercise and cardiac health education) for people who have a primary cardiac diagnosis, including myocardial infarction, coronary artery bypass graft surgery, valve surgery, acute coronary syndrome, Transaortic Valve Replacements and angioplasty.
Main duties of the job
- You will be part of a skilled Cardiac Rehabilitation Team and provide the core components of Cardiac Rehabilitation
- You will provide patients with a personalised assessment tailored to their condition and need
- You will provide Cardiac Rehabilitation to include exercise, education and psychological support, both face to face in gym sessions and via telephone for a Home Programme
- You will work with patients to identify risk factors and set goals
- You will have a passion for empowering patients in reaching their goals and use motivational interviewing to facilitate this
- You will support the implementation of a patients plan, assessing progress regularly along their pathway
- You will encourage engagement with all aspects of Cardiac Rehabilitation
- You will identify symptoms and respond to or escalate these as appropriate
- You will support the ethos and vision of our Cardiac Rehabilitation Service and deliver the core components of Cardiac Rehabilitation and liaise with others within the team, Clinical Nurse Specialists and Cardiology Consultants
- You will be involved in the Audit of patients under Cardiac Rehabilitation Services
