We are looking for enthusiastic health care assistants with an interest in caring for frailty patients, to join our specialist Frailty team in Hereford County Hospital. Our FSDEC (Frailty Same Day Emergency Care) opened in September of last year and has played a major role in improving the journey of patients through the WVT frailty pathway. Patients who are admitted to our FSDEC are discharged via a number of pathways, including discharge back to their own home within 24 hours, to community hospitals for support with rehabilitation or on to our specialist frailty wards for further management.

We take pride in providing high quality, specialist care for our frailty patients and ensuring they are cared for in the right place, by the right people at the right time.

Main duties of the job

The main duties of the role of a health care support worker on the FSDEC is to support the Registered Nurse with providing a high standard of patient care, providing personal care, helping patients with their dietary and fluid intake, helping them use the toilet and to assist with repositioning.

Good communication skills are essential in this role , alongside the ability to document and escalate any worries to the trained staff
