This post will attract a Recruitment and Retention Premia** totalling £5,000 (payable in instalments over 2 years), plus a relocation package of up to £8,000 (subject to eligibility criteria). The RRP payment will be pro-rata for part-time employees.

We are looking for an experienced and enthusiastic Pharmacist with an interest in Respiratory. The post involves leading the provision of medicines optimization to our Respiratory teams in a varied and dynamic division.
The successful candidate will work closely with the ward based teams implementing the principles of the modern day ward round, using skills, knowledge and experience to directly impact on patient care.
Main duties of the job
The post holder will:
- Be the senior specialist respiratory pharmacist across the One Herefordshire Health economy.
- Work as part of the multidisciplinary respiratory team (MDT) and across the ICS, Acute hospital team, and Primary care teams to develop and promote pathways and guidelines for patients in line with national and local guidelines.
- Typical working week:o Pharmacy operational: Ward based (Arrow ward and respiratory patients) & Dispensaryo Specialist Asthma hub MDT (monthly)o Respiratory business / educational meetings (monthly)o Member of Medical Gas committee (bi monthly)o Working with Primary Care Pharmacy teams as link to Respiratory Specialtyo Training and Education.
- To work in collaboration with Acute Hospital Specialist Respiratory team reviewing respiratory patients in acute care, and domiciliary settings, providing support as a Clinical Pharmacist and as a non-medical prescriber, including to the specialist severe asthma clinic.
- To lead on evidence-based prescribing advice to ensure effective, appropriate, sustainable, cost-effective and safe use of medicines and promote patient-centred care for all patients.
- To lead on guidelines for the prescribing, administration and storage of respiratory related medicines to facilitate good practice with respiratory care, within the Acute Trust and across the primary and secondary care interface.
