An exciting opportunity has arisen for a highly motivated Community Nursery Nurse to join an innovative team delivering the Healthy Child Programme to young children and their families within Herefordshire.

The successful candidate will work under the supervision and leadership of the health visitors. The post will require you to have excellent assessment and communication skills and you should demonstrate a strong commitment to partnership working and supporting children and their families. You will also be expected to demonstrate a sound knowledge and understanding of child safeguarding.

As a community Nursery Nurse within the team you will receive a robust package of clinical, safeguarding and restorative supervision as well as additional training required for the post.

There may also be instances whereby the successful candidate will be required to work out of other bases in order to meet the needs of the service. Access to a car and a driving license is a requirement for this post as travelling is essential for the role.

Main duties of the job

  • To undertake developmental screening in line with the Healthy Child Programme as delegated by the Health Visitor. Contribute to the appropriate referral process to other agencies and resources as required.
  • To deliver and evaluate an appropriate behavioural modification programme (in line with local standards) in collaboration with the named Health Visitor.
  • To utilise "play" to optimise a child's developmental opportunities, maximising this time to involve family/ carers members in this process.
  • To participate in Health Visitor Clinics, giving appropriate advice on topics such as nutrition, breast feeding and behavioural problems and make appropriate and timely referrals, informing the Health Visitor of the referral.
  • To work collaboratively with the Health Visitor and multi-agency professionals in the delivery and evaluation of Care Plans that have been jointly agreed with parents/carers/ young people
  • To provide support and advice to individuals, carers, families and wider communities within own areas of expertise.
  • To participate in wider public health activities as identified by the Practice or Operational Lead.
  • To participate in Health Promotion and Education programmes for individuals and groups, coordinating resources and promotional materials.
  • To undertake designated work as directed by the named Health Visitor/ School Nurse with vulnerable children and families including those subject to a child protection plan.
