Are you seeking a new challenge or looking to work in a different care setting with exposure to new opportunities? We are offering an exciting position for a Band 6 Junior Sister to join the Care Coordination Team to work in our Hospital Inreach Service.
Main duties of the job
The Care Coordination Inreach Team work across our acute and community sites to help streamline patient discharges. The team visit the acute site daily (New Cross Hospital) attending a wide range of wards to identify patients who will need community support on discharge and take the lead with discharge planning for identified patients. Our aim is to achieve a safe, timely and seamless transition from hospital to home for our patients. Candidates must have a passion for service improvement and be motivated to promote, deliver and ensure success of the inreach service. Experience in service improvement and leadership is desirable as the post will involve the successful candidate working autonomously to implement, promote and deliver the inreach service with measurable set outcomes throughout the 12 months.
Applicants must be confident to promote safe and timely discharges into the community and lead with discharge planning (including liaising with MDT, patients and their families).
Candidates must have a UK driving license, business insurance and own a car they can use for work purposes. The post will require frequent travel across community sites and to New Cross and West Park Hospitals. Applicants must have a live NMC registration.
