Job summary

Wolverhampton Health Visiting Service is undergoing exciting service development and expansion resulting in the creation of some new posts, as part of this we are looking for an enthusiastic and dynamic SEND Health Visitor to join our 0-19 Health Visiting Service.

Are you an inspirational and highly driven experienced Health Visitor or newly qualified Health Visitor who has exceptional leadership and clinical skills and want to make a difference to our SEND health visiting workforce and SEND families of Wolverhampton?

Car owner/driver essential and you must be able to meet the travel requirements of the post.

Main duties of the job

To understand local needs and priorities in relation to SEND (special educational needs and disability) in under 5 year olds and understand how they apply to, direct and influence HCP (Healthy Child Programme) provision.

To deliver a programme of child centred care to children under 5 years with cognitive, social, and emotional difficulties, physical disabilities, and sensory impairment.

To provide support and guidance to nursery/ education settings (and potentially other professionals) on designated caseloads

To work with the Health Visiting Team and IASS (Information, advice and support service for special educational needs and disability) and partners in education including the DMO and DCO (designated medical officer and designated clinical officer).

To facilitate the transition of children and young people in various settings when entering and leaving the caseload

To support the CDC (Child Development Centre) by attendance at monthly panel and liaising with professional and services as needed.

To work with families, community groups and professionals to ensure consistent advice is offered in Wolverhampton and assist in the delivery of excellent local and specialist services to improve the health and wellbeing of our local families.

Job responsibilities

As a Health Visitor you will individually and within a team be accountable for the management and delivery of programmes of care for a designated caseload/population.

Ensure the delivery of clinically effective practice through the effective utilisation and integration of evidence based practice and implementation of evidence based standards of care, policies, procedures and protocols.

Ensure a culture of continuous quality improvement through the use of audit, patient/family/stakeholder feedback and reflection on practice by self.

Ensure the identification of the health needs of families and communities, using appropriate methods and tools.

Promote a clean and safe environment for staff, children and families by ensuring compliance with legislation, policies and protocols including health and safety, healthcare associated infection prevention, clinical and information governance, including risk management, critical incident reporting and root cause analysis.

Ensure effective communication with the multi-professional team, statutory voluntary organisations and other partners to ensure all care given is appropriate to the child and family and to prevent duplication of services.

Ensure a high standard of care and record keeping in accordance with Nursing and Midwifery Council, national legislation and local standards.

Report incidents and near misses promptly and appropriately and take effective action to minimise future risk.

Ensure an effective and efficient child and familys journey by planning and co-ordinating the episode of care including the smooth transition to other settings, promoting effective communication with interdisciplinary and interagency teams as required.

Identify vulnerable families and children at risk and participate in safeguarding activities, looked after children reviews, child in need and CAFs as appropriate to promote positive health and prevent deterioration of the child and familys health and wellbeing.

Assess and interpret clients conditions, deliver and evaluate health visiting interventions. Influence clinical decisions and monitor the quality of child and family care provided through clinical knowledge relevant to own field of practice, underpinned by theory and experience.

Within a multidisciplinary team environment, develop and embed a culture of family-centred care placing the child at the centre.

Be highly visible within the service, communicating regularly with children and families and/or carers; promote a caring environment where equality and diversity issues are respected and families are enabled to be partners in their care.

Identify opportunities to develop care and services for the client groups.

Ensure that everyone within the sphere of care is treated with dignity and humanity, understanding individual needs, showing compassion and sensitivity, and provide care in a way that respects all people equally.

Maximise all opportunities to contribute to the health education/ promotion agenda and principles of every contact counts

Identify local public health needs by analysing and utilising public health data and work with the community to build community capacity
