Job summary

This is an exciting secondment opportunity for a Band 6 nurse to join our ambitious, friendly and supportive Children and Young People in Care Team. This specialist nurse post will be suited to someone wanting to be part of a service offering high quality, proactive support and intervention to enhance the health and wellbeing of our children and young people. We need an individual who has determination and a passion to meet the health needs of our children and young people, putting their voice at the heart of service provision.

Main duties of the job

To manage and co-ordinate the assessment, planning, delivery and evaluation of healthcare provision for our children and young people in care, completing statutory review health assessments.

To provide high quality care and health promotion to children and young people in care.

Assist in the development, delivery and evaluation of specialist training to foster carers, young people, the wider multi agency team and partner agencies.

Develop effective multi agency working relationships to improve the health outcomes and life chances of Children and Young people in care.

Advocate for Children and Young People ensuring their voice is always heard and at the heart of everything we do.

Undertake health assessments for children and young people in care within a 50 mile radius of Wolverhampton.

Support and advise colleagues in the clinical assessment and care of children and young people, whilst being clear about others personal clinical professional accountability.

Provide advice and signpost to other professionals about legal processes, key research and policy documents.

Work with the named and designated professionals on planning and developing strategy for services for children and young people in care.

Ensure advice is available to professionals trust-wide on day-to-day issues about children and young people in care and their families.

Engage in appraisal, support and supervision for colleagues in the team and organisation.

Work harmoniously and effectively with colleagues, patients and clients and their carers, families and friends, seeking advice from senior practitioners as necessary.
