Job summary
The Royal Wolverhampton NHS Trust Primary Care Network (RWT PCN) is looking for a skilled Information & Performance Lead to manage their Information and Performance Team.
This varied and rewarding role is an excellent opportunity to help shape the future of primary care as a key member of RWT PCN, which consists of nine GP practices. The aim being to enable provision of proactive, personalised, coordinated and more integrated health and social care for patients, closer to home.
This role is required to provide an expert point of reference for timely, high quality information including trend analysis, trajectory reports, assessment and action planning of variances and support onareas such as Quality Outcomes Framework (QOF), performance management and data collection.
We are looking for an enthusiastic and motivated candidate to manage the Information and Performance Team, managing tight deadlines and communicating with a variety of staff across the PCN. The role will be very rewarding as the post holder will be using and interpreting data to make a difference to patient services, via operational teams and data submissions to external organisations.
The Information and Performance Lead will support the Primary Care Network in enabling staff to readily obtain the information they require to optimise clinic utilisation and monitor demand and capacity through the PCN'sclinical system and business intelligence tools.

Main duties of the job
To assist Primary Care Services Information Manager, RWT PCN GP Service Delivery Managers and Practice Managers with clinical information management, data quality and implementation of service improvement plans and delivery of organisational objectives.
To validate and extract data, conduct monitoring, data analysis and produce reports and recommendations using various internal and external data resources.
To provide relevant, timely, high quality information including trend analysis, trajectory reports, assessment, and action planning of variances.
To line manage staff within the team and ensure the team functions efficiently and effectively to meet business needs.
To build effective working relationships with a range of stakeholders across the organisation and the wider healthcare community.
To communicate through a variety of media, face to face and virtually.
