Staff Nurse - Same Day Emergency Care

Job summary

This is an exciting opportunity to join Acute Medicine at SaTH. We are a fast paced unit with 8 trolleys & 14 chairs. As a band 5 nurse within our Same Day Emergency Care (SDEC) unit, you will play a key role in delivering high-quality, patient centred care to individuals requiring urgent assessment, diagnosis & treatment without the need for hospital admission. You will work as part of our dynamic multidisciplinary team, supporting the rapid assessment & management of patients presenting with a wide range of acute medical conditions.

Your role will involve undertaking timely clinical assessments, administering treatments, monitoring patient progress, and providing clear information & reassurance to patients and their families. You will be expected to demonstrate sound clinical judgement, prioritise workload effectively & respond to changing patient needs in a face-paced environment.

You will also contribute to the timely flow of patients through the unit by collaborating closely with doctors, ACP's and administrative staff, ensuring care is safe, efficient and in line with national SDEC pathways & guidance.

Main duties of the job

• Responsible for the assessment, planning, delivery and evaluation of individualised patient care, working in partnership with patients, their carers and the multi-disciplinary team to ensure the patient's physical, spiritual and emotional needs are met.
• Formulate and deliver high quality patient care without direct supervision, seeking guidance on actions that are outside agreed defined standards.
• Recognise changes in patients' conditions which require the intervention of others and ensure that timely referrals are made or escalation procedures enacted as appropriate.
• Ensure patient safety is maintained at all times. Record and report adverse and potentially adverse events and assist with investigation of such events, ensuring learning is shared with the wider team(s).
• Record information about patients in a safe, accurate and consistent manner using appropriate tools and techniques, including electronic and written formats as close to point of care as possible.
• Access and use patient clinical diagnostic information to inform clinical decisions.
