Job summary
This post provides a great opportunity for someone with healthcare experience to join our community Dietetic team in Shropshire.
A dietitian supports people with their nutrition and diet both in the hospital and in the community. An assistant practitioner supports the qualified dietitians and holds their own delegated caseload. The post holder will be working in the community visiting the community hospitals, care settings and patients in their own homes supporting patients who require dietary advice , additional nutrition via supplements or via feeding tubes.
The post holder will be either visiting patients or phoning them in clinics. This role will also support the team with any administration or computer system work.
The post holder will be trained on how to use the feeding pumps and to support patients to use them in their own home. There will also be an educatory role
Main duties of the job
Be responsible for own caseload of Home Enteral Feeding patients within Nursing Homes and provide support to other appropriate patients with the supervision from the Dietitians.
1. To assess and implement treatment plans for a specific patient group.
2. To hold responsibility for own caseload with access to and support from a qualified Therapist.
3. To carry out the assessment of patients within a specific diagnostic group. In partnership with the patient, identify needs, devise objectives, provide and deliver an individualised treatment programme, within an agreed protocol and progress treatment plans.
4. To hold responsibility for own caseload, working without direct supervision within the remit of the post. Formal Supervision takes place regularly and access to advice and support from a qualified Therapist is available at all times, clinical work is routinely evaluated.
5. To work within agreed protocols and own competencies referring cases that progress to be complex back to the Therapist or to continue after advice has been given. (please refer to Candidate job pack for further duties of this role)

The community team consists of a 4.4 w.t.e qualified dietitians and another band 4 Assistant Practitioner.
The community team forms part of the wider dietetic service provided across Shropshire and Mid Wales consisting of the acute team at both Royal Shrewsbury Hospital and Princess Royal Hospital and the specialist teams.
