We have 30 hours available for a part time Dementia Support Worker who will support the Dementia Clinical Specialist in the delivery of improvements to Dementia care across the organisation.
The successful applicant will take a key role in undertaking the role of ensuring good dementia care across both hospital sites as well as working alongside frontline staff, to support the care of patients with dementia, their families and carers.
Applications are invited from experienced Health Care Support Workers who have experience of caring for patients in an acute care setting and have a Level 3 NVQ or are prepared to work towards.
You will be passionate about Dementia care, demonstrate the fundamental values that drive our Trust, possess excellent communication skills with proven ability to work as part of a team as well as being able to work independently.
Main duties of the job
To organise their daily workload, to ensure all people living with dementia receive good dementia care in each area, to achieve this you will be promoting butterfly scheme, REACH approach, hospital passports, support plans, use of abbey pain scale, partnership work regarding nutrition and hydration, screening assessments as required, communication with wider MDT, families/carers. To carry out administration tasks in support of the work e.g. photocopying, scanning, PSAG boards, ward vision, Clinical Portal, vitalpac and use of an iPad for your caseload.

- Level 3 Diploma in Clinical Healthcare Support or NVQ Level 3 in Care or demonstrate a commitment to work towards the Diploma qualification
