Job summary

The Shrewsbury and Telford NHS Trust has an exciting opportunity to welcome a new Locally Employed Doctor (ST4+ Equivalent) in Renal Medicine. This is an exciting opportunity for a Junior Doctor to progress their skill set and gain valuable experience working in an established team.

Main duties of the job

  • Be a part of a consultant lead clinical team providing specialty and General medical care to medical inpatients.
  • To support on-call in-patient ward cover team.
  • To develop enhanced clinical skills, especially in the assessment and management of acutely ill patients.
  • To have excellent team-working skills with the abilities and professional attitude to work well in a multi-professional team.
  • To deliver clinical and quality targets agreed both nationally and locally with clinical teams and commissioners.
  • To follow patients throughout their clinical pathways.

Both hospitals provide a wide range of acute hospital services including accident & emergency, outpatients, diagnostics, inpatient medical care and critical care.

Job description

Job responsibilities

The Department

The renal inpatient beds are situated on a 18 bed ward Ward 35. There are Consultant ward rounds on a Monday, Wednesday and Friday morning and board rounds Monday Friday morning. There is a Friday 4:00pm handover to the weekend covering Consultant. All renal Consultants have equal access to junior doctors. We have 2 Speciality Doctors who will support the renal wards depending on the staffing levels of junior Doctors.

Additional support for the renal service includes a nurse operational manager, modality sister, renal anaemia coordinator, home haemodialysis sister, vascular access sister, pre-end stage dialysis sister, renal transplant sister, and 2 dieticians. At RSH site we also have a West Midlands Renal SpR, 2 Specialty Doctors, an IMT3, CT2, CT1, FY1,a GPVTS trainee, a FY3 and a physician associate. Weekly multi-disciplinary meetings, a monthly renal forum, monthly governance , monthly histopathology meeting and fortnightly educational meetings also take place. There are fortnightly dedicated surgical day-case access lists provided by the three vascular surgeons who have demonstrated a clear commitment and interest in vascular access surgery. Two consultant radiologists provide renal radiological services including vascular access surveillance, MR angiography and renal artery stenting.

The Haemodialysis Unit

The Haemodialysis unit at The Royal Shrewsbury Hospital provides chronic haemodialysis for over 90 patients (20 stations). On Monday, Wednesday and Friday there are three dialysis shifts whilst on Tuesday Thursday and Saturday there isnt a twilight shift. The renal unit also provides haemodialysis for approximately 80 patients per year with acute kidney injury. A 16-station satellite haemodialysis unit at the Princess Royal Hospital currently provides dialysis for 90 patients. The satellite haemodialysis unit at Ludlow currently caters for 16 patients. There are plans in expanding the Satellite unit capacity to a 31 station centre, catering for the planned growth of dialysis patients in the next decade.

Renal Transplants

The majority of our renal transplants are performed at the Queen Elizabeth Hospital, Birmingham with whom there are close links. Patients return for post-transplant care at three months. There are currently around 180 renal transplant recipients under our care. Our combined renal- pancreas transplants are usually done at University Hospital, Cardiff. Future expansion will include the earlier repatriation of transplant patients, requiring an increased transplant work force.

The Peritoneal Dialysis Unit

The Peritoneal Dialysis unit is based on ward 35 and is managed by an enthusiastic PD nursing team who care for 60 peritoneal dialysis patients. The number of PD patients has nearly doubled in the last 3 years and is expected to continue to increase. Our peritonitis rates have traditionally been low. Approximately 70% of patients receive APD. Acute PD and assisted APD are also provided depending on clinical need. We provide a medical PD insertion service for patients who are deemed suitable. The home therapies teams have been recently expanded.

The Home Haemodialysis Program

The home haemodialysis program has an excellent record and continues to flourish and expand. It is headed by 3 senior nurses and 1 HHD sister. Currently 30 of our patients receive their dialysis at home. This number is expected to increase further. We were one of the first units in West Midlands to use NxStage machines for HHD and currently we have 24 patients using this system.

With an expanding home therapies service, the clinical workload will be shared between three consultants. As a result, clinics for PD and HHD patients will increase in number.

The Transplant Listing Team

The transplant listing team (including the transplantation sister, transplant link nurses and 2 dedicated transplant lead consultants) have ensured that we actively partake in the transplant first programme and are working hard to increase our live donor transplant rates as well as our pre-emptive transplant rates. There are ample opportunities for the new appointees to contribute to the transplant service, particularly as there is an ambition to achieve earlier repatriation of transplant patients from the transplanting centres.

CKD Care

CKD care has been streamlined with the development of a low clearance clinic. Four consultants , working with the CKD team manage over 250 patients. Dialysis and transplantation preparation, as well as palliative care are managed within this service. SaTH has an excellent record of access preparedness for dialysis (>70%+ per year achieving the Renal Association dialysis access standards).

SaTH has further developed CKD services and activated a CKD detection system. With the development of informatics resources, CKD referral from Primary care has improved. Late presenting patients with ESRF has fallen from 25% to 5% per year. Over 3000 CKD patients are now managed in general nephrology clinics. The new appointees will contribute to these duties. Further developments in managing CKD in the community is envisaged.
