Job summary
We are looking for an enthusiastic pharmacist to join a friendly, progressive clinical team, serving patients at West London NHS Trust.
The successful candidate should be a highly motivated individual and a team player with good communication skills and a keen interest in mental health. The applicant should be able to work well both in a team and autonomously.
An applicant who starts at Band 6 would be expected to work towards the Postgraduate Certificate in Pharmacy Practice in order to progress to Band 7. An interest in mental health would be a requirement.
An applicant who starts at Band 7 would need to hold the Postgraduate Certificate in Pharmacy Practice. A postgraduate Certificate in Psychiatric Pharmacy or Clinical Pharmacy Diploma would be desirable, but training can be supported. Previous experience of working in and/or interest in mental health would be a requirement.
Main duties of the job
Rotating through Hammersmith and Fulham, Hounslow, and St Bernard's Sites, initially based at St Bernard's, the post holder will work closely with the pharmacist (Advanced) on each site and the multidisciplinary team to optimise pharmaceutical care for patients.
They will provide a clinical pharmacy service to allocated wards, offering advice to medical staff, nurses and patients on all aspects of the safe & effective use of medicines as well as facilitating medicines reconciliation and discharge planning. They will also support the delivery of education, training and service improvement using quality improvement methodology.
