An exciting opportunity has come up for the Tissue Viability Service. We are currently looking for a Band 5 Tissue Viability Community Nurse to work across the Hounslow areas based at Heart of Hounslow Centre for Health, Bath Road. The service operates from 0900 - 1700 Monday to Friday.
Collaboratively working with our District Nurses, primary care, integrated rapid response teams, community matrons and social service colleagues are essential to ensure seamless care to our patient group.

Main duties of the job
To provide high quality, skilled nursing care in Tissue Viability (TV) and to assist the Tissue Viability Nurses in the on-going planning, implementation, and evaluation of the holistic needs of the patient/client, family, and/or carer.
To lead and manage the Leg Ulcer clinic at multiple sites with the support from the TV lead nurses.
To work jointly with the Practice Nurses, community nurses, Pressure Ulcer Prevention Nurse, and other healthcare providers in relation to wound care.
Offers/performs clinic appointment and home visits.
