Job summary
As a Clinical Pharmacist, you will be a self-motivated individual who works well independently and as part of a team.
You have an exciting opportunity to play a crucial role in supporting, promoting and implementing the high quality, evidence based and cost-effective use of medicines in primary care in a way that maximises the benefits of medicines to patients and service users, and which minimises risks (clinical, legal and financial).
You will also support the development, and implementation of agreed prescribing and pharmaceutical activities or programmes within GP practices across a PCN
Main duties of the job
You will have the opportunity to run face to face clinical medication reviews and to be part of a team that sets up and runs long term conditions clinics for patients with complex medication regimes. You will also understand the pharmacy role in governance and be able to implement this appropriately within the workplace.
You will enjoy this role if you are a highly motivated and skilled self-starter, with the ability to provide specialist input on person centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services.
