Job summary

An exciting opportunity has arisen for an exceptional and highly experienced candidate to provide strategic and operational leadership focused upon supporting inpatient flow and safe discharge planning for inpatients.

This is a fixed term one-year contract. The post holder will be expected to provide leadership to the health and social care agenda within inpatient services across Hounslow and H&F and ensuring timely and proactive discharge planning for service users.

The successful candidate will be expected to develop and maintain close and positive relationship with key stakeholders, lead and coordinate work with a range of professionals and provide professional expertise on relevant health and social care legislations.

This role comes at an exciting time when we have the development of new services and are working on piloting a number of improvement initiatives across the AMHS Service Line. Your role will be pivotal in supporting the development and implementation of these transformation initiatives. Additionally, you will provide leadership and support embedding these new initiatives in practice.

Main duties of the job

o Provide expert leadership to the health and social care agenda within inpatient services, ensuring timely and proactive care planning is offered to users and their families to the best possible standard in keeping with the Mental Health Act 1983, Mental Capacity Act 2005 and the Care Act 2014.

o Develop and maintain close and positive relationship with key stakeholders to ensure timely discharge planning to include the 3rd sector community offer and housing.

o Promote a strengths-based patient-centred approach to discharge planning, keeping the principles of independence, choice and control at the heart of service delivery.

o Work alongside Clinical Inpatient Services and Community Services to identify barriers to discharge at point of admission and seek to implement and engage services to address these barriers, so that no patient is in hospital longer than necessary.

Essential

  • Professional qualification in Social Work and registered with Social Work England plus relevant experience and training
  • Masters or equivalent CPD portfolio
  • BIA Qualified

Desirable

  • Approved Mental Health Professional (AMHP) or demonstrable practice and expertise in an Adults Inpatient service and knowledge of mental health legislation.

Experience

Essential

  • Extensive post qualifying and management experience in adult mental health services.
  • Significant experience of social work leadership.
  • Experience of working across complex organisational boundaries.
  • Experience of managing different organisational cultures and working across professional groups across Health and LA's.

Knowledge

Essential

  • Extensive knowledge, experience and understanding of delivering high quality and cost effective services for service users.
  • Detailed knowledge of professional and regulatory standards and skills in applying them in practice including complex situations.
