Job summary

With West London NHS Trust's planned service development of its community rehabilitation service in the three boroughs it serves, Hounslow, Ealing and Hammersmith and Fulham, the team is looking to recruit a dynamic, creative and hardworking Band 6 Care Coordinator for a fixed term contract of 12 months to support the service delivery of its rehabilitation pathway for referred service users, to support them to achieve their recovery goals and live the best life possible.

Main duties of the job

The team work closely with service users with complex psychosis and their carers, to help maximise independent functioning within the least restrictive setting possible and actively promote recovery. This role will involve care coordination and working to develop detailed CPAs, risk assessment and management plans, health and wellbeing plans, and the provision of evidence based interventions in individual and group settings. There will be opportunities to participate in and potentially lead on quality improvement initiatives and research.

In this role you will work closely with the wider clinical team, inpatients services, Early Interventions Service, community mental health teams, MINT, the supported housing sector as well as a wide variety of local 3rd sector organisations.

  • Registration must be live with a professional body such as NMC or other relevant body

  • Family Therapy/Work training
  • Other relevant training, e.g. substance misuse

Experience

Essential

  • Experience of working in multidisciplinary mental health team with clients with severe and enduring mental health problems

  • Supervision of students and junior staff.
