Job summary

West London Trust (WLT) is a dynamic healthcare organization committed to delivering high-quality care to our community. We are currently seeking a dedicated and experienced Service Manager to join our Integrated Care team based at Clayponds Hospital.

As the Service Manager for Bedded Rehabilitation, IC, you will play a pivotal role in overseeing and coordinating community-bedded step up/down pathways at Clayponds Hospital. This includes managing P2, Neuro/Rehab and Admission Avoidance beds.

Main duties of the job

The role of Service Manager for Community-Bedded Pathways entails providing both strategic and operational leadership to ensure seamless transitions of care for patients. This involves managing the delivery of community-bedded pathways, ensuring interventions are timely and effective. Collaboration with multidisciplinary teams is crucial to develop and implement innovative strategies aimed at improving access to care and reducing hospital admissions. Additionally, the Service Manager leads service development initiatives, working closely with stakeholders to address any gaps in service provision. Ensuring compliance with regulatory standards and best practices is paramount, along with monitoring service performance and implementing quality improvement measures as needed. Lastly, providing leadership and support to staff is essential, fostering a culture of continuous learning and development within the team.
