Job summary
An exciting opportunity has arisen for a senior manager who has extensive experience in managing community health services to join us as Head of Unplanned Care. We are seeking to recruit a high calibre individual with excellent communication skills; a clear commitment to overseeing the delivery of high quality care to the local population in a changing environment and experience of delivering services in a transformational and innovative way.
The successful candidate will be expected to work closely with system partners both at local and NW London ICB level and will have the skills to develop and foster relationships across organisational boundaries.
The post holder is a key member of the Integrated Care Service Line senior team and is pivotal in the provision of effective leadership in ensuring that the priorities for the service line are delivered.
Main duties of the job
The post holder will work closely with the Clinical Lead for Unplanned Care to ensure that high standards of care are maintained throughout the services.
The portfolio includes a large community rehabilitation hospital, Urgent Community Response Teams, Community Rehabilitation Teams and Community based Referral Hubs.
The post holder will provide line management support to the Service Managers and will be responsible for providing assurance that services are delivered to the expected standards including financial, performance and workforce.
They will be expected to lead on the development and implementation of transformation plans for their portfolio and play a strategic role across the Trust in relation to developing Community Services in line with the Trust's Strategy. They will also be expected to contribute to the developments of integrated care at a borough and NW London level.
