Job summary

Being part of the Mental Health Single Point of Access (MH SPA) at West London NHS Trust, you will be at the forefront of innovation of service delivery in one of the fasted growing parts of community healthcare.

The right candidate will be responsible for the overseeing of all referrals into the service and determining the appropriate care pathways our patients. You will also lead and monitor the performance and work of the staff in the team and will work closely with the team manager and deputise for them when required.

Additionally, we work very closely with the Crisis Assessment and Treatment Teams and there will be the expectation that you will rotate in to the CATT in order to obtain further experiences of managing patients in an acute mental health crisis face to face and also to ensure we provide you with well-rounded opportunities.

Main duties of the job

The post holder will be responsible for screening and triaging referrals received. They will provide clinical expertise in supporting Telecoaches and Mental Health Advisors in making decisions to ensure that patients are transferred into the appropriate secondary or primary mental health services, where there is an assessed need for that service. Where screening and clinical triage indicate that specialist mental health care is not indicated, support staff in facilitating a safe discharge back to Primary Care with advice.

The post holder will be responsible for creating an environment which is safe, supportive and an easily accessible with a no wrong door approach and will also undertake brief crisis assessments, give advice, information, signpost and use their expertise to de-escalate presentations or interface with other services for more appropriate or intensive input/follow-up.

Qualifications & Training

Essential

  • Registered RMN with eth NMC, or be a Registered OT, Social worker (HCPC).
  • Evidence of ongoing professional development
  • Mentorship/Teaching & assessing qualification

Desirable

  • First or higher degree in appropriate subject area.
  • ENB 998, 812 or equivalent
  • Nurse prescriber or willingness to become a nurse prescriber

Experience

Essential

  • Experience in a variety of MH settings as a qualified practitioner.
  • Significant experience at band 6 or equivalent within CRHTT, Liaison nursing, intake service or equivalent.
  • Delivering Clinical supervision/ reflective practice to nursing as well as other professionals.
  • Multi-disciplinary team working.

Desirable

  • Previous experience of working within a Single Point of Access services
  • Previous experience of telephone based assessments.

Knowledge & Skills

  • Understand a range of models of mental disorder, including the contribution of social, physical and development factors
  • Understand the social perspective on mental disorder and mental health needs.
