Job summary
The role of the Diabetes Nurse Specialist is to develop and maintain relationships and networks with groups, individuals, MDT within Forensic services.
To act as a source of clinical knowledge and expertise to those with diabetes, their carers, health care professionals and other statutory and non-statutory agencies and to work as part of the Primary Healthcare team as directed in delivering services.
Main duties of the job
Basing own practice on evidence of good practice in diabetes and nursing care, leading evidence based improvements and enabling others to apply them in practice.
- To work closely with the Integrated Diabetes Service and other members of the multidisciplinary team including lead consultants, GP's, nursing colleagues, other health care professionals, social services and voluntary sector services.
- To hold and manage an active caseload.
- To undertake patient assessment, develop, implement and evaluate plans of care for patients with diabetes
- To participate in nurse led clinics, outreach and community clinics and to undertake home visits as required
- To contribute and support the education strategy as agreed with the designated work stream facilitator and Nurse Consultant/Team Leader
- Undertake clinical procedures and interventions commensurate with role, including initiating and adjusting insulin doses as indicated and recommended oral diabetic medication in accordance with national and local guidelines/practice
- Provide telephone support assessment and advice to people with diabetes on their management in accordance with national and local guidelines/practice

- The collection of statistical data and activity as required/requested to contribute to e.g. performance report
- To attend regular meetings with the Nurse Consultant/Team Leader to undertake PDP and appraisal
- To attend and contribute to the Integrated Diabetes Service and multi-disciplinary team meetings across the primary and secondary care setting
- To represent the Integrated Diabetes Service and/or Specialist Services at appropriate meetings locally, regionally and nationally.
- To assess and support local GP practices and provide specialist support to improve the standards and delivery of diabetes care
- To support and up skill GPs & Practice Nurses in delivering Level 1 & 2 care in their clinical networks
- To maintain a clinical case including patients who are housebound
- To promote national guidance and policy such as NSF standards and proposed changes to working practices in order to identify patients with diabetes and improve their care.
- Contribute to the implementation of policies, standards and guidelines within the Integrated Diabetes Service.
- To participate in WLT Nursing Forums.
- To ensure effective risk management at team level by accident/incident reporting, assessing and controlling risk and ensuring residual risks are added to the Trust Register LEADERSHIP
