Job summary

The service comprises a fully integrated mental health team with a visible presence in the local Primary Care Network. There will be a tiered or stepped care model within the MINT team where a more primary care ethos and model of delivery will be used for the majority of patients but those who have more complex needs will have more enhanced support from an allocated clinician as well as the multidisciplinary team. The post will therefore involve consultation/liaison with the MDT as well as GPs, primary care services and other mental health teams. Alongside this there will be some direct consultation with patients particularly around complex cases and crisis prevention.

Our implementation of MINT has taken a whole-systems approach to the way Mental Health care is delivered with transformation at every level of service and care delivery.

This has involved integrating primary and secondary care teams and reconfiguring community teams so they are positioned to deliver care in alignment with the local primary care networks and work in partnership with local communities. Furthermore, by combining resources from different sectors and recruiting link workers to build connections with VCSE offers, MINT is able to support people to access community and social groups and develop networks with local communities. This approach seeks to tackle health inequalities and in so doing improve outcomes for mental health and physical health needs.

Main duties of the job

This is an exciting opportunity to lead an innovative new integrated community mental health service. You will play an integral role in further developing the service, teaching and training new team members and primary care staff and providing clinical and managerial leadership to the team.

Based in the community, MINT aims to deliver joined-up, personalised treatment and care which support and are responsive to, the needs of the local population. MINT teams are place-based; aligning primary care networks across the boroughs to deliver an integrated approach to mental health, physical health and social care needs. By working closely with GPs, social services, and voluntary sector partners across the local community, MINT offers timely access to integrated care. The service vision is 'The right care. At the right time. In the right place.'

MINT offers a range of therapeutic interventions which support and are tailored to individual needs. These are delivered in both group and one-to-one settings. The services and support available throughout MINT differ across each borough, reflecting the needs of the local population in each area. A stepped approach to care is taken, with brief group interventions offered as a starting point ensuring a range of accessible treatments including therapeutic support, education and training, employment, volunteering and other occupational support, peer support, and link workers building community connections

Job description

Job responsibilities

Clinical leadership: You will have a key role in a liaison/consultation model to provide early, easy access advice to GPs and the Primary Care Networks to support care of patients in the locality. GPs in the borough will be able to contact you via telephone or email for advice during working hours. You will also provide clinical leadership to the team in clinical multi-disciplinary team meetings and referrals meetings as well as being available for case discussions, advice and supervision within and outside team meetings. You will also have a role in the management of a smaller number of more complex cases, to offer senior assessments and more direct input in particular in risk and/or medication management and diagnostic assessments. This will also include Mental Health Act Assessments, CTO reviews and supporting the duty function of the team with urgent concerns and assessments. You will have senior medical responsibility for the care carried out by the MINT team.

Relationships: you will be expected to establish, develop and maintain positive relationships with colleagues in the Primary Care Network as well as in secondary care mental health teams, IAPT, Local Authority and 3rd sector. You will also facilitate smooth transitions into and out of the service through relationships with other relevant services.

Service development: You will work in partnership with the Team Manager to provide leadership and development to the MINT Team. This will include developing strategy, quality improvement, audit, and written policies and guidelines for the future delivery of MINT Services through working collaboratively with others in WLNT and the PCNs and other stakeholders.

Training and education: You will be expected to participate in training for key individuals in the Primary Care Network such as GPs as well as within the MINT team itself. This may include formal didactic teaching; individual bedside style clinical teaching based around individual cases; developing strategies for teaching and training. You are also likely to contribute to complex case forums across disciplines, some GP practice or locality meetings and other community fora to improve clinical care and communication across services.

You will also be expected to contribute to the academic programme and Teaching/ training/junior supervision.
