We are looking for a Band 8a Lead Diabetes Specialist Nurse, with Non-Medical Prescribing and recognised diabetes qualifications.
The post holder will provide the strategic vision, expert clinical leadership and management to develop, implement and evaluate the diabetes nursing service in West London Forensic services.
The role will require advanced clinical skills and experience. The post holder must be able to instigate and review diabetes treatment for both type 1 and type 2 diabetes. You must be able to adjust insulin according to protocols and titrate medicines safely and accurately in line with Diabetes guidelines, policies and NMC standards.
You will educate GP's, Practice Nurses, ward doctors, nurses and AHP's in supporting their patients with diabetes and working collaboratively with MDT in weight management and prevention of diabetes. You will demonstrate strong decision making skills with the ability to motivate other non- specialist practitioners into improving their knowledge in the management of the person with diabetes. You will also provide structured education programmes for people with both Type 1, Type 2 and pre-diabetes.
You will be working in Primary Health care, West London Forensic services, in three different buildings within St-Bernard's Hospital site and will also be required to do ward visits.
The service is still being developed and you will be integral to suggesting new ideas for service improvement and innovative ways of working.
Main duties of the job
- To work closely with members of the multidisciplinary team including lead consultants, GP's, nursing colleagues, other health care professionals, social services and voluntary sector services.
- To hold and manage an active
- To undertake patient assessment, develop, implement and evaluate plans of care for patients with diabetes
- To participate in nurse led clinics, outreach and community clinics and to undertake home visits as required
- Undertake clinical procedures and interventions commensurate with role, including initiating and adjusting medicines including insulin doses as indicated and recommended oral diabetic medication in accordance with national and local guidelines and scope of practice.
- Provide telephone support assessment and advice to people with diabetes on their management in accordance with national and local guidelines/practice.
- To act as a resource for other professional groups, teams and individuals providing care for people with diabetes and their families
- To act as a resource for patients with diabetes and their families and establish follow up systems to provide ongoing support
- To advice and facilitate newly diagnosed people with diabetes to appropriate services
