As one of the 12 'early implementer' sites for integrated community mental health services, we are expanding our community mental health offer. Our new services are centered around the Primary Care Networks and aspiring to comprehensive engagement with Integrated Care Systems using a whole person, whole population health approach.

The model utilizes all the community resources available including health, social care, Voluntary Community and Social Enterprise (VCSE) organizations and local communities and consider resources to support wellbeing outside of mental health services alone.

These new teams enable and support people to access mental health services wherever and whenever they need it and support individuals to manage their mental health supported in their local community.

We are therefore looking for skilled and enthusiastic individuals.

There really has never been a more exciting time to join West London NHS Trust.

Main duties of the job

The post holder will lead and monitor the performance and care delivery of the nursing staff in the MINT team; ensuring local professional leadership. They will participate in review of clinical policies and guidelines relevant to the area and support and implement changes to practice.

Additionally, they will liaise with other organizations and professionals to promote communication and good practice; be able to undertake audit activity and supervision to other staff in service area/care group. They would need to have an understanding of psychosocial interventions to include psycho-education, wellness recovery action plans and discharge planning.

The post holder will also be responsible for the assessment of care needs and the development, implementation and evaluation of programs of care for a small and defined caseload; and maintaining associated records.
